This report was prepared by the Division of Population Surveys in the Office of Applied Studies (OAS), Substance Abuse and Mental Health Services Administration (SAMHSA), and by RTI, Research Triangle Park, North Carolina. At SAMHSA, Douglas Wright was responsible for the content, analysis, and writing of the report, and Joseph C. Gfroerer reviewed and provided numerous suggestions and improvements on its content and format. At RTI, Ralph Folsom was responsible for the overall methodology and estimation for the model-based Bayes estimates and prediction intervals. The following staff were responsible for generating the estimates and providing other support and analysis: Akhil Vaish, Neeraja Sathe, and Lea Drye. Lisa Packer, Teresa R.
